The first NHL season in 1917–18 was contested between 4 teams, all of which were located in Canada:
Montreal Canadiens
Montreal Wanderers
Toronto Hockey Club (now the Toronto Maple Leafs)
Ottawa Senators (original 1883-1935 franchise)
So there was not always teams in 2 countries.
The term "Original Six" is probably the reason for the perception that there were always both American and Canadian teams in the NHL. There were many now defunct teams in the NHL before the Original Six (Toronto, Montreal, New York, Boston, Chicago and Detroit).
The term Original Six is actually referring to the 6 "original" teams in the league prior to the 1967 expansion when the league added 6 additional teams. They were not the 6 first teams in the league.
